A pioneer in the field of refractive surgery, Dr. William Boothe was one of the first cornea specialists to be trained in vision correction techniques during his Fellowship. In addition, he was the first cornea-trained surgeon in the Dallas area to use an FDA-approved Excimer laser for refractive surgery. Dr. Boothe was among the first three surgeons in the Dallas area (and one of the first in the country) to use a microkeratome, the instrument that creates the flap during the initial part of ALK and LASIK surgery. The delicate procedure of precisely lifting the flap is critical to overall success of the surgery.

The laser is programmed prior to each procedure for the ablative "cut" based on the patient's pre-operative refractive error (nearsightedness, farsightedness, and astigmatism). Once the laser ablation is completed, the surgeon gently replaces the corneal surface flap to restore surface integrity of the eye. It's as if the refraction is "ground in" to the middle layers of the cornea! The results are often dramatic and very rapid, with most patients seeing well enough to drive a car without correction the very next day. However, the best post-operative visual acuity may not be obtained until 2 to 3 weeks, or in some cases, even a few months, after the procedure.

A native Texan, Dr. William Boothe received his undergraduate degree from Rice University with honors and completed his residency at Texas Tech University School of Ophthalmology. Dr. William Boothe attended the University of Texas Southwestern Medical School in Dallas, before serving an internship at Presbyterian Hospital in Internal Medicine. Dr. Boothe completed a cornea fellowship at Jules Stein Eye Institute (University of California Los Angeles Medical School) where he specialized in refractive surgery.

Acute angle-closure glaucoma: Angle-closure glaucoma is due to an obstruction of the system that drains aqueous fluid from the inside of the eye. As a result, fluid accumulates and the pressure within the eye increases. Patients usually have a tendency to develop angle-closure glaucoma because of crowding of the anatomy of the front of their eye. It is more common in people who are hyperopic (farsighted). This disorder may be triggered after an eye exam in which the pupils have been dilated or by taking certain oral medications in the susceptible individual. Common symptoms include a severe headache or eye pain accompanied by nausea and vomiting. Vision is also usually blurry. Angle-closure glaucoma should be suspected if these symptoms develop after an agent is used to dilate the pupils for an eye exam. Artificial tear drop: Lubricants (also called artificial tears) are synthetic (manmade), water-based solutions that are used to lubricate the eye and thicken tears. Artificial tears are formulated as solutions or suspensions, varying in viscosity. Popular examples of artificial tears include AquaSite, Bion Tears, Celluvisc, Duratears, Gen Teal, HypoTears, Liquifilm Tears, OcuCoat, Refresh, Systane, Tears Naturale, and TheraTears. Many people develop sensitivity to the preservatives in these solutions, causing increasing redness, burning or itching. Most of these products are also available in a preservative-free (PF) form. Artificial tears usually are used two to five times a day as needed for relief of symptoms.
Current surgery for presbyopia includes Laser assisted in situ keratomileusis, or LASIK, and CK, Conductive keratoplasty. These techniques all give the patient monovision so it is better to try out monovision with contacts before surgery, especially in the case of LASIK where changes to the cornea are permanent. LASIK involves permanently changing the shape of the cornea in one eye allowing better near vision while leaving the other eye alone. LTK and CK use different techniques but both aim to change the shape of the cornea using thermal conduction of specific areas in the cornea. Optometrist - (ahp-TAHM-uh-trist) A doctor of optometry (OD), which is not a medical degree. An optometrist diagnoses vision problems and eye diseases, and prescribes glasses, contact lenses, and drugs to treat disorders. They provide post-surgical care, but do not perform surgery. The education is at least three years and most optometrists have a B.A. or more. This is followed by four years at an optometry school, with an examination at the end. Contact dermatitis: Contact dermatitis is an inflammation of the skin that causes swelling, scaling, or redness of the eyelid with intense itching. If both the upper and lower eyelids are affected, the cause of the inflammation is likely to be an allergic reaction. LASIK EYE - DR WILLIAM A. BOOTHE => A phakic intraocular lens (PIOL) is a lens implanted inside the eye for the correction of either extreme nearsightedness or extreme farsightedness. In effect, the lens becomes an internal contact lens rather than contact lenses on the surface of the eye. It is usually recommended for patients whose visual correction is outside the range that can safely be treated with LASIK surgery. Because of the slightly increased risk of more serious complications, the PIOL is reserved for high amounts of nearsightedness or farsightedness--above the current limits of LASIK. In places where this technology is available, surgeons are implanting the PIOL in patients with myopia greater that 12.00 to 15.00 diopters and hyperopia greater than 4.00 to 6.00 diopters. Despite the excellent outcomes in most cases, complications associated with the implants are currently the biggest concern. Specifically, in the early studies a small percentage of patients developed cataracts shortly after implantation of the lens. There is also a small risk of endophthalmitis (infection within the eye) because the surgical incision actually enters the eye. This rare complication could lead to complete loss of vision. Endothelial cell loss with some lens designs is also a concern and is being studied rigorously. The Versyse Lens is already FDA approved and some Ophthalmologists in the United States are currently performing this procedure with other lenses as part of an FDA clinical trial. The procedure holds a lot of promise for extremely nearsighted or farsighted individuals. Ophthalmologists are anxious to see how the implantable lenses fare in current studies using newer lens designs and implantation techniques. These lenses are currently being used in Europe and South America with very high success rates. The results will be presented to the Food and Drug Administration with the hope that the FDA will authorize other eye surgeons to use this exciting new technology.
Dystrophy - Weakening or wasting of body tissue, such as happens in Epithelial Dystrophies and Stromal Dystrophies, where abnormalities appear in different corneal membranes, causing loss of function and often impairing vision.

DR. WILLIAM BOOTHE - DALLAS => Orthokeratology involves first measuring the refraction of your eye which tells the doctor the amount of nearsightedness, farsightedness and astigmatism you may have. Next it will be necessary to map the shape of the front surface of the eye using and instrument called a corneal topographer. Based on the digital map of your eye and the amount of your prescription to be reversed, the eye doctor will develop an exact shape and set of parameters necessary to prescribe in order to obtain the desired effect on your corneal shape. The special contacts are manufactured to exact specifications for your eyes. They will initially be worn for about 8 hours daily until proper correction is achieved and your vision is improved. Sometimes, your doctor will actually prescribe a wearing schedule whereby you will only need to wear the lenses while you sleep. After that, the lenses may only need to be worn a few hours a day for about 3 days a week to keep the cornea in the shape necessary to allow you to see clearly. The exact wearing schedule really depends on your eyes and correction and will be determined by your doctor. Upon removal of the lenses you will be able to obtain improved vision without any corrective lenses. This is a great alternative for those who are too young to consider LASIK, those whose prescriptions are continuing to change and is especially useful for youngsters and teenagers involved in sports where wearing contact lenses might be limiting in their sports activity. A new type of lens and material actually allows accelerated ortho-k that can produce results in days rather than months.

Presbyopia is part of the normal process of aging. As a person becomes older, one begins to lose the flexibility of the lens of the eye which limits the ability of the eye to change its point of focus from distance to near.

Pupil - a hole in the center of the iris that changes size in response to changes in lighting. It gets larger in dim lighting conditions and gets smaller in brighter lighting conditions

Macular Edema - Pooling of fluid in and around the macular area of the retina, which causes swelling and impairs vision. It causes blurriness, waviness of straight lines, light sensitivity, and a pink tint to the vision. It usually happens as a result of disease or injury and sometimes after eye surgery. Most people recover from it in a matter of months.

There is also Pseudoesotropia, which looks like esotropia but is a temporary condition where a young child’s eyes haven’t yet grown the white part next to the nose. The bridge of the nose appears wide and the eyes appear to be both turned inward. This resolves itself as the child’s face develops.

The goal is to achieve a more optically perfect eye, though the final result still depends on the physician's success at predicting changes which occur during healing. In older patients though, scattering from microscopic particles plays a major role and may exceed any benefit from wavefront correction. As the light continues further into the eye, passing through the pupil to reach the lens, it’s refracted more, to a variable degree according to how far away from the eye the object is which is reflecting this light. If it’s down the road a ways, like a distant traffic light, it needs to be refracted less in order to land in focus on the retina.
